Have you blocked any instances? I believe if you block an instance and get a reply or message from someone in that instance it shows the replay as an unread notification but hides the actual message/reply

Never blocked an instance...and to be honest don't know how because haven't join any other instance besides lemmyworld. And I don't think I will.

You dont need to join another instance to get comments from that instance. Instances are federated and share content between them. You see content via your instance which come from other federated instances - unless you view solely “local” posts
Are you using an app, or a browser? ~~I'm wondering if you have some open reports from the communities you moderate.~~ Apps generally don't handle reports well, and some mix them in with private messages.
Edit: Nevermind, I checked the reports queue. Still, it would be helpful to know what interface you are using.
