It is very likely that the jihadists had planted forces all over Northern Syria. Then they attacked Aleppo, and when the Syrian Army counter-attacked (very successfully at first), the hidden jihadists were activated behind their lines and basically cut off all communications and supply.
The Syrian Army had to retreat behind Homs to save the situation. However, it should be noted that this retreat was performed mostly without a fight.
There's also a good possibility that this entire operation came too soon. The urgency of Israel to sign a ceasefire with Lebanon point to this. It is very likely that Trump's election made them act quickly and perhaps (hopefully) prematurely.
