The character create has you choose your pronouns and one of the options is they/them
You can also pick your genitalia independently of your body type.

Is this actually a hentai game?
There's an option when you first start the game to enable "cinematic nudity".
If you toggle it on, there are some scenes where the protagonist is naked and a handful of sex scenes that you have to go out of your way to encounter.
All the genitalia option during character creation does is pick what's on display during these scenes.
Not with some of them.
