My sex assigned at birth in my medical records has been female for a few years now, and suddenly after my latest appointment (where they even said they're just doing organ inventory now!) it's been changed to male. I have of course changed it back.
They can literally just switch it
Sex is immutable folks when you can just switch it with a couple clicks:

When I was sent to the hospital for wanting to Jill myself they originally gave me a band that said M on it, and then a lady came and changed it to one with an F and I have no idea if she knew I was trans or thought I was a girl and it was some kind of error.
